INTRODUCTION
PURPOSE
This document will detail how MAHAJAYA PRO RECYCLING SDN BHD carried out the decommissioning of Faulty Network Equipment together with data and power cables at the customer premises. This method statement covers all the work carried at the customer premises. This document constitutes the Written System of Work for the implementation (site survey, decommissioning and disposal) in a safe, timely and effective manner with minimal disruption to normal working.
HEALTH & SAFETY / GENERAL INFORMATION

- On Site Daily Risk Assessment - to be completed on a daily basis before work commences
WORKPLACE
- All site safety requirements will be observed as laid out in the customer guidelines.
- All unused materials and waste will be removed on completion unless otherwise stated.
- The workplace will be subject to a Site Survey prior to work commencing in order to identify any hazards peculiar to the workplace. Should any risk be identified which falls outside of normal operations, our Project Manager will contact his Project Engineer for further instructions.
- A safe passageway and means of safe access shall be designated and kept free of obstacles in order to enable workers to transfer equipment to the loading vehicle from the building and onto the apparatus floor.
- Safety barriers and signage shall be erected in any area that may require them in order to warn and protect people from possible exposure to hazards arising from work carried out by the workers.
- Any dust and debris created during the transfer/siting activities shall be cleared up as the decommissioning progresses.
- Upon completion and on leaving the site at the end of all activities, all temporary barriers and signage erected by the workers shall be removed.
AUTHORISATION AND PERMITS
- Any authorization / permits to work at site issued by customer has been obtained by our Project Manager.by writing to our customer upon their request in advance.
- Our Project Manager reported to the customer’s assigned representative where appropriate. In the case of unmanned sites, access was arranged by Mahajaya Pro Recycling with the customer prior to arrival.
- It is the responsibility of the customer to ensure all appropriate work permits used by its organisation are issued to the Project Manager and his workers.
- It is mandatory for Mahajaya Pro Recycling’s Project Manager and his team to comply with all requirements set out by site rules and any other additional requirements.
- Our Project Manager is responsible for access and control of the work.
COMPETANCY
The following competencies have been identified as a requirement for this project.
- All site workers with Manual Handling Training
- All site workers with Basic Health & Safety Training
- Certified Electrician by Energy Commission of Malaysia
SUPERVISION
The following personnel is responsible for the supervision of health and safety, environmental and quality management on site throughout the completion of the project.

EQUIPMENT / TOOLS
The following tools that were used:

8. Lashing Rope or Cable Tie

- All tools are to be checked regularly for damage.
- Tools which are subject to wear and tear are replaced.
- Low voltage or portable battery-operated equipment used where possible.
PERSONAL PROTECTIVE EQUIPMENT (PPE)
Specific sites will present general industrial hazards and PPE must be worn as required by Site Rules. All our workers are provided with the following PPE:
- Steel toe-capped footwear
- Hard hats / safety Helmets
- Eye protection
- Face dust masks
- Hi-Viz Jackets
- Hand Gloves
WORKING AT HEIGHT (ABOVE HEAD LEVEL)
Overhead access may be required for some cables decommissioning activities.
The use of ladder is considered the most practical access method to overhead cabling.
Ladders are subject to pre use checks each day by our workers.
Our Project Manager will contact the owner and inform him / her of the problem if access to overhead areas is required, and is not achievable through normal routes (i.e: the use of a ladder). The job will then be reviewed between Mahajaya Pro Recycling and the customer and a suitable strategy will be implemented.

EMERGENCY ARRANGEMENTS
- All our workers will follow on site emergency procedures.
- In the event of an emergency, our Project Manager will notify the Person in Charge at TOC of the occurrence as soon as he is able to.
MANUAL LIFTING
All lifting of racks / servers will be carried out manually by our trained workers. A two men assisted Pallet Hydraulic Jack will be used for loading purposes without damaging the floor.
ENVIRONMENTAL INFORMATION
MANAGEMENT OF WASTE
Mahajaya Pro Recycling has its own procedures to ensure the legal and ethical management of waste streams.
The customer’s waste streams will not be used unless prior agreement has been received.

WORK PROCEDURES
ARRIVAL ONSITE
Upon arrival to the site, our Project Manager will report to the site contact.
PREPARATION
- Our qualified electrician will ensure there is no power connection from the main supply prior to the decommissioning taking place.
- Our project Manager will ensure that the conditions of the areas in which they are working are free from basic observable hazards prior to commencing work.
- Any unusual conditions will be reported to the owner in the first instance.
DECOMMISSIONING
- Decommissioning will be carried out in accordance with the agreed project plan and scope of works. All permitted conditions will be adhered to.
- At the end of each day, the worksite will be made safe and any permits will be returned to the customer’s site representative.
- The Decommissioning Team will review the site survey, site risk assessment, health & safety plan, any method statements, generic activity-based risk assessments, PIP documentation and work instruction prior to arrival on site. If there are no known changes the implementation team will proceed to site. If changes are necessary the implementation team will take the appropriate steps and proceed to site.
- Upon arrival on site our Project Manager will use all reasonable endeavors to make himself known to the designated site contact and book onsite with the controlling party (where required, dependent on local site rules).
- Our Project Manager will be responsible for access and control of the work.
- Our Project Manager will check with the customer’s site representative that there is no deviation from the site survey.
- Decommissioning Team review Site Risk Assessment form to ascertain if there are any changes to the risks/hazards present.
- Decommissioning Team performs an “Unpack and Check”, reporting any issues to the Project Manager immediately.
- All reasonable efforts will be made to minimize risk from materials or working practices to Mahajaya Pro Recycling or customer’s personnel.
- Decommissioning team will perform the decommissioning.
DISPOSAL
Our well-trained general workers will lift the decommissioned equipment on the trollies and send them up to the loading point before loading them on the lorry for disposal without damaging the floor.
